Stereotactic Cisternal Lavage Therapy in Patients with Aneurysmal Subarachnoid Hemorrhage with Urokinase and Nimodipine for the Prevention of Secondary Brain Injury. A Randomized Controlled Trial. (SPLASH)

Aneurysmal subarachnoid hemorrhage is a severe stroke that affects relatively young people (mean age: 55 years). Delayed cerebral infarction is a unique complication of this disease and contributes heavily to its morbidity. Delayed infarcts are due to constriction of blood vessels wich is caused by blood break-down prodcuts. Effective therapies are lacking.
Stereotactic catheter ventriculocisternostomy is a novel treatment concept which enables removal of cisternal blood. Our Initial experience was highly promising: Introducing this method into clinical practice led to a decline of delayed infarcts by more than 50% and a reduction of delayed infarct burden by two thirds.
Assessment of safety and efficacy of this new therapy in a randomized trial is the aim of this research project.
